Why does the number on my prescription always go up?

I'm in law school and have had glasses since grade school, so I know the higher number means I got more nearsighted and need thicker lenses. What I want to know is, why do my eyes keep getting more and more nearsighted? I do everything I'm supposed to do -- look in the distance regularly, avoid dim lighting, hold books as far from my face as possible, etc. But none of this has stopped or slowed my progression. Recently, my prescription increased from -12 to -13. I want to prevent it from going to -14 next time. Please tell me what to do.
