What is the normal cause of myopia getting worse in adulthood?

If a person's prescription gets stronger during the 20s and 30s, is this because the retina is getting further back from the lens? Or is it typically that the lens of the eye is losing its ability to assume a long focal length?
Added (1). Discounter: You just contradicted your answer to my other question. I asked whether wearing long-distance glasses while reading will make the condition worse, and you said "yes."
