Eye doctors and/or Optometrists. Can you please explain to me how an eye exam works (Refraction)?

So I know you look at a chart 20 feet away and read a row while they put different lenses in front of you ("1 or 2?").

My question is, what row are you supposed to be looking at when the doctor does this? Are you supposed to be looking at the topmost row or the bottom?

I'm nearsighted (have trouble with distance) if that helps.
