I think i might have type 2 diabetes?

Im hungry all the time.
Im getting a bit chubby even though im eating the same amount of food i always eat.
White stuff around my genitals, but i dont wash there so i found out its something called "segma?"
Im tired at school towards the end of the school day but when i get home and during the day im not tired.
At one point in maths last period i felt like i was falling asleep.
I have blurry vision, Shortsightedness.

But im still very very worried and scared to go to my parents (im 12) :(
Added (1). I also sometimes the past few days have had a very small pain around my heart area.
Added (2). Sorry i meant during the holidays im not tired.
