Contact lenses? How to read prescription?

So I recently lost my glasses and couldn't get another pair from my eye doctor because of my insurance policy. I wanted to get contacts because I have an event coming up soon and really need to be able to see. When I want to order contacts online, how do I fill out the questions based on this prescription? Thank you.
